RE-ENTRY


If you are a EMT-Basic or EMT-Intermediate and you are unable to renew your license because you did not
complete any or some of the renewal requirements, you will have to take the following steps:

1. Complete a Registration for Training/Licensure Application front and back. This form can be downloaded from the website. (click here)

2. Attach a copy of a current refresher course completion certificate at your licensure level (completed within the last 9 months).

3. Attach a copy of a current healthcare provider CPR card.

4. Attach a check, money order or PO (NO cash accepted) for the appropriate re-entry fee:

EMT-Basic $ 90.00
EMT-Intermediate $120.00

Once we have received your complete re-entry packet Ute Fennicks will schedule you for the next available state test site
to complete your NM EMS state exam. A confirmation letter will be mailed to you.
